This questionnaire provides us with the information to make changes to your current CIBMTR reporting. If you wish to completely stop reporting data, please go to the Center Withdrawal or Closure Questionnaire HERE. Note: Under U.S. law, U.S. centers performing allogeneic transplants must report them.
Be sure to get to the end of the questionnaire and get a confirmation of the information you have submitted for your records. Further work on this issue will be done via a CIBMTR Center Support.
We strongly recommend completing this questionnaire on a computer rather than a mobile device. Be sure to fill out the entire questionnaire. We are only notified that you have completed it when you reach the last page of the questionnaire.
